Authorization: I (we), submitting this form below, do hereby authorize FiberTel, Inc. to charge the above numbered credit card for the amount filled in above ($100 Minimum). This charge represents a prepayment of the month's estimated Callback usage. FiberTel, Inc. will track and deduct all chargeable calls from the monthly prepayment until 70% of the prepayment is utilized (30% remains), at which time FiberTel, Inc. is again authorized to charge the above card in the amount necessary to reinstate the estimated Callback prepayment. This authorization is to remain in effect until FiberTel, Inc. receives written notification from the undersigned to cancel the authority. Upon receiving notice of cancellation, FiberTel, Inc. will refund any unused portion of the prepayment, less a 4% credit card refund charge. I am personally responsible for all charges relating to the below listed Callback lines. I understand that I will receive a monthly statement with call detail including dates and charges. I further understand that in the event my credit card becomes invalid, that I will provide FiberTel, Inc. (or its designee) with a valid credit card number upon request or pay in cash or money order any and all outstanding balances owed to FiberTel, Inc. I understand that my credit card will be charged for anticipated usage and that all charges will appear as FiberTel, Inc. on my bill. Disputes must be sent in writing (return receipt requested) no later than 10 days after receipt of invoice to: Customer Service, FiberTel, Inc., 7808 El Cajon Blvd. Ste M, La Mesa CA 91941. To terminate service, the subscriber must provide written notice to FiberTel, Inc. at the address above. By signing below, I understand that I am responsible for all charges made on the FiberTel, Inc. long distance service and agree to pay all FiberTel, Inc. charges on the above prior to written notice. Due to the nature of the services being offered, FiberTel, Inc. may terminate at any time. I agree that any dispute will not be cause for withholding payment and that I must pay all invoices in full, regardless of any disputes being negotiated. All credits, if any, issued for resolution of dispute will be applied to the current billing cycles' invoice in which the dispute is resolved. Subscriber is responsible for paying all applicable taxes. FiberTel, Inc. does not extend any implied warranty or fitness of services for a particular purpose with respect to quality, pricing, and availability of services.
